PAYMENT POLICY
St. Bernard Eye Care's services are provided and billed by our healthcare office. Our providers participate in most all of the major health insurance plans in the region. Following are the health plans which we currently accept.
If your plan is not listed, we will be unable to file a claim on your behalf. However, we have prices available for insurances that we are not providers for or for uninsured patients.
Please be aware that with any health plans, there may be deductibles and copayments for which you could be responsible. Fees will be rendered at the time of your visit.

St. Bernard Eye Care has two parts of this business. The Exam side and the Glasses Department.
The Opticians accept Healthy Blue for all ages to receive an exam. However, Healthy Blue does not cover the glasses material for adults 21 and older. This would mean that the glasses are the patients responsibility and are not covered by Healthy Blue.
We are a provider for Medicare unless it is attached to People's Health, WellCare, or Take Charge.
We accept Blue Cross Blue Shield for medical visits such as irritation or an eye disease, however if the vision plan is Davis Vision, the routine eye exam will not be covered and it will be the patients responsibility.
We accept Blue Advantage plans unless the vision is Davis Vision. The Routine Eye Exam would be the patients responsibility.
QUESTIONS ABOUT YOUR BILL
What billing or insurance information will I receive?
It is expected for each patient to have a general understand of their insurance plan. If a co-payment is required, we ask that you please bring this amount on the day of your visit as fees are due when services are rendered.
How long will it take to get things settled with the insurance company?
Generally, it takes from 45 to 60 days to obtain payment from an insurance carrier.
Why am I getting bills from St. Bernard Eye Care?
If the patient filed insurance and the insurance plan only covered a portion or percentage of the visit, the remaining amount is the patients responsibility.
If an insurance claim was denied due to cancellation of insurance or unmet deductibles, the patient would be responsible for the payment for services rendered.
I received a notice that my insurance company has paid on my bill -- but I can't understand how they calculated their payment amount. Do you know?
If we have received any such information from your insurance company, we'll be glad to share it with you. However, for answers to any questions about insurance payments, deductibles, or co-payments, you generally need to check with your insurance company.
