At Eyedox, we offer a wide range of services billed directly through our healthcare office.
We’re proud to participate in most major health insurance plans in the region. Below, you’ll find a list of the plans we currently accept.
If your plan isn’t listed, we won’t be able to file a claim on your behalf—but we do offer flexible pricing options for patients without insurance or with out-of-network plans.
Please note: depending on your specific health plan, you may be responsible for deductibles or copayments, which are due at the time of your visit.

What billing or insurance information will I receive?
It is expected that each patient has a general understanding of their insurance plan. If a co-payment is required, we ask that you please bring this amount on the day of your visit, as fees are due when services are rendered.
How long will it take to get things settled with the insurance company?
Generally, it takes from 45 to 60 days to obtain payment from an insurance carrier.
Why am I getting bills from Eyedox?
If the patient filed insurance and the insurance plan only covered a portion or percentage of the visit, the remaining amount is the patient's responsibility.
If an insurance claim is denied due to cancellation of insurance or unmet deductibles, the patient would be responsible for the payment for services rendered.
I received a notice that my insurance company paid my bill -- but I can't understand how they calculated their payment amount. Do you know?
If we have received any information from your insurance company, we'll be glad to share it with you. However, for answers to any questions about insurance payments, deductibles, or co-payments, you generally need to check with your insurance company.
